How to start the conversation

Not a confrontation. Not in front of others. Not while they are under the influence. Choose a private, calm moment and lead with what you feel, not what you accuse: 'I am worried about you and I don't know what to do.' Silence is fine. Their first reaction is rarely their real one — give them time and expect a defensive response you do not answer.

If they refuse treatment

This is common, and it is not the end. Change often comes after the third or fourth conversation, not the first. Your job is not to force a decision today — it is to make it clear you will not disappear, and that a door is open when they are ready.
