How to help someone who refuses treatment

You cannot force someone into recovery. You can, however, dramatically increase the odds that they get there — and much of the research on this is under-used.

The evidence on family involvement

The CRAFT model — Community Reinforcement and Family Training — has decades of research behind it. Families trained in CRAFT succeed in getting a resistant loved one into treatment in roughly two-thirds of cases, and reduce their own distress meaningfully along the way. This is not about pressure; it is about a specific skill set.

What CRAFT actually teaches

How to reinforce non-using behavior without infantilizing. How to withdraw reinforcement from using behavior without cutting the person off. How to time a treatment conversation for maximum openness. How to look after your own life while you wait. All of it teachable, all of it non-confrontational.

What not to do

Do not stage a public intervention without professional preparation — the classic 'confrontation' model has poor outcomes and often destroys trust. Do not pretend nothing is happening. Do not threaten what you cannot follow through on. Do not lie to the person about what you know.
